Complete if you wish to withhold Directory Information from the public.

Des Moines University does not permit access to, or the release of, educational records and personally identifiable information without the student’s written consent and request, other than to officials of the institution and to those granted access by the Family Educational Rights and Privacy Act.

The following information is considered to be Directory Information, and is not subject to the above restriction of public access or release: name, local address, telephone number, DMU email address, major field of study, year in program, dates of attendance, enrollment status, degrees and awards received and participation in officially recognized activities.

If you wish to have your Directory Information withheld from the public please complete and submit this form to the Office of the Registrar. After the signed form is submitted, the Directory Information restriction will remain on your record until you notify the Office of the Registrar in writing that the restriction should be lifted.
