GENERAL SUMMARY: AGE OF POPULATION SERVED ESSENTIAL JOB FUNCTIONAL COMPETENCIES: Demonstrates the knowledge and skills necessary to provide service/care appropriate to the age of the patients served, in accordance with Hawarden Regional Healthcare standards. Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Compliance Accountability Program, Code of Ethics, and HRH policies and procedures, including all provisions of the Health Insurance Portability Act of 1996 (HIPAA). In order to ensure adherence in a manner that reflects honest, ethical and professional behavior by implementing the organizational vision, values, and mission. Performs Occupational Therapy (OT) evaluations for all age groups of patients. Occasionally, performs evaluations for adolescents, children, and pediatric patients Demonstrates ability to interpret physiciansโ referrals. Can identify precautions, contraindications in less common diagnoses. Obtains all pertinent information from the patient chart prior to initial evaluation, including significant past medical history, present medical surgical interventions. Consults with physicians, other caregivers and higher level staff for further clarification as needed. Recognizes and requests when further tests or consults may be beneficial for more thorough patient management. Reviews chart prior to treatment. Consistently recognizes information which requires alteration of OT intervention. In more complicated situations, seeks guidance from appropriate personnel. Incorporates data obtained into treatment session. Chooses and utilizes appropriate evaluation procedures (uses those procedures as defined by each specialty area). Demonstrates ability to assess general mental, emotional and cognitive status. Recognizes abnormal subtle changes and makes appropriate recommendations to attending physician for further consults. Usually recognizes abnormal physiological or psychological signs and takes appropriate action. Reports these to M.D. and appropriate personnel. Seeks guidance from M.D. and other medical personnel when needed. Sets measurable appropriate goals based on estimated length of hospital stay, projected length of OT. Seeks occasional assistance from higher staff for more complex diagnoses. Revises treatment plan and goals as appropriate for basic and more complicated diagnoses. Utilizes patient care protocols for evaluation procedures. Makes recommendations for new procedures. Utilizes subjective and objective evaluation results to plan appropriate treatment program which incorporates components of strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance and gait for functional abilities and engagement in daily living activities. Educates patient, nursing, and family in their participation with the Occupational Therapy program. Involves patient and family as appropriate in treatment planning and discharge. Includes recommendations for discharge. Aware of Occupational Therapy resources in the community and makes discharge recommendations regarding these. Participates in individual patient care team conferences, rounds for purposes of coordinating a patientโs plan of care. Makes verbal reports regarding patientโs status. Recommends equipment for patientโs hospital and discharge needs using most appropriate and cost effective materials. Utilizes appropriate Occupational therapy interventions, procedures and equipment for patient treatment as defined by the specialty area. Frequently utilizes research findings in evaluation and treatment. Is responsible for checking that department equipment is in working condition and meets safety standard prior to patient treatment. Reports defective equipment to Therapy Director. Is responsible for written documentation that includes initial evaluations, progress notes, home programs, discharge summaries, statistical reporting, purchase orders, interagency forms, and incident reports that are legible, inclusive, concise, accurate, grammatically correct, timely and meet standards of department/hospital, CAH, outside regulatory and insurance agencies. Provides written materials as appropriate as an adjunct to verbal instruction and utilizes audiovisual resources in the department to enhance instruction. Independently organizes schedule to reflect patient care priorities. Recognizes needs of fellow staff on unit. Arranges for patient care duties to be assigned during absence. Carries patient load that reflects productivity standard of the unit. Provides education to department members within one to two months after attending a continuing education course (when MMC provides funding to attend). May include teaching an in-service program; providing educational materials; initiating a special project; or developing a policy, procedure, or protocol. Presents a review of a research article pertinent to patient care one time every year. Supervises assigned OTAs and technicians and assists Therapy Manager in completing the performance appraisals. Must have a general working knowledge of computers and department specific software (i.e. Healthstream, internet, etc.) Actively participates in departmental meetings, committees, conferences, and in services. Is knowledgeable of and complies with Safety and Infection Control Policies & Procedures. Participates in Continuous Quality Improvement activities as requested. Keeps up to date on new procedures and research. Maintains confidentiality of information pertaining to clients, physicians, employees and HRH business. Completes all mandatory training such as Healthstream, The Patient Experience, Safe Choices, Crucial Conversations and any other training as designated by Administration.
