Job Title: Physician, MD/DO

SUMMARY
The Physician provides comprehensive, patient-centered medical care within their scope of training and specialty in a clinic and/or hospital setting. This role may include primary care or specialty services depending on assignment. This Physician is responsible for evaluating, diagnosing, and treating patients while promoting preventive care, managing acute and chronic conditions, and coordinating care across the healthcare continuum. This position supports WCHCโ€™s mission to deliver high-quality, accessible healthcare to the community.

DUTIES/RESPONSIBILITIES (include but are not limited to the following)
Clinical Care
โ€ข Provide medical care appropriate to specialty, including preventive, acute, and/or chronic disease management
โ€ข Conduct patient assessments, histories, and physical examinations
โ€ข Diagnose and treat a range of conditions within scope of practice
โ€ข Order, interpret, and communicate laboratory and diagnostic test results
โ€ข Perform procedures appropriate to specialty and credentialing
โ€ข Prescribe medications and develop treatment plans
โ€ข Manage chronic conditions such as diabetes, hypertension, COPD, and heart disease
โ€ข Provide age- and condition-appropriate screenings and preventive services
Care Coordination & Continuity
โ€ข Coordinate care with other physicians, specialists, hospitals, and care providers
โ€ข Facilitate referrals and ensure appropriate follow-up care
โ€ข Participate in transitions of care and population health initiatives as applicable
โ€ข Collaborate with interdisciplinary teams to optimize patient outcomes
Patient Experience & Communication
โ€ข Educate patients and families on diagnoses, treatment plans, and prevention strategies
โ€ข Foster a patient-centered environment that promotes understanding and trust
โ€ข Build and maintain therapeutic relationships with patients
Documentation & Compliance
โ€ข Maintain accurate, timely, and complete electronic medical records
โ€ข Document patient encounters, care plans, and outcomes
โ€ข Comply with regulatory, billing, and quality reporting requirements
โ€ข Review and sign charts per organizational standards
Hospital & Community Engagement
โ€ข Participate in community health initiatives and outreach as appropriate
โ€ข Support organizational quality improvement and performance initiatives
Leadership & Organizational Contribution
โ€ข Provide clinical guidance to nursing and support staff
โ€ข Collaborate with advanced practice providers and care team members
โ€ข Participate in committees, meetings, and organizational initiatives
โ€ข Recommend process improvements to enhance patient care and efficiency
โ€ข Promote WCHCโ€™s mission, values, and standards
โ€ข Display and promote company values
โ€ข Regular attendance
โ€ข Other duties as assigned
