SUMMARY:Innovation Summit: Collaborative Care Models
DESCRIPTION:Innovation Summit 2013: Collaborative Care Models is a live 1-day event to attend virtually. You are able to attend one session or the entire day!It will bring together leaders in the health care profession and\, specifically\, the physical therapy profession to explore integrated models of care such as accountable care organizations (ACOs)\, patient-centered medical homes (PCMHs)\, and bundled payment\, and the role of physical therapy\, with the goal of developing strategies for bolstering the involvement of physical therapists in these models.Participants will be able to watch live as their colleagues\, physicians\, large health systems\, and policy makers who are actively participating in and at the decision-making helm in these models discuss the role of physical therapy and the future of health care. As a virtual conference attendee you will be able to incorporate these innovative ideas into an action plan for integrating new models of care into your practice.We also encourage you to participate through Twitter at @APTAtweets using hashtag #PTSummit.Register online Objectives Identify the latest developments in the formation of integrated financial delivery systems under the Affordable Care Act.Differentiate between accountable care organizations (ACOs)\, bundled payment models\, and patient-centered medical homes (PCMHs) and address the challenges and opportunities posed by each for physical therapists.Implement quality improvement\, patient-centered\, and population health initiatives into your practice and articulate your practice’s value in integrated models of care.Adapt your practices to address clinical and regulatory challenges and opportunities that arise regarding the implementation of innovative models of care such as ACOs\, PCMHs\, and bundled payment models.Apply new administrative and clinical processes to collect outcome data and form relationships with large health systems and physician groups to improve quality and lower costs.Assess current implementation of innovative models under Medicare\, Medicaid\, and commercial insurers and implement successful strategies to participate in these programs.Identify trends in private payer and employer-sponsored plans regarding direct access and increased focus on the value of physical therapy.Delivery MethodBlended Course: This course will require you to access the APTA Learning Center online to download handouts and view the presentations.
