How do I obtain a copy of my transcripts, verification of my degree or certification for licensure purposes or to acquire staffing privileges?
You can fill out our online request form for dean's letters, verification, transcripts, board scores and letters of recommendation OR, contact the Registrar's Office, 3200 Grand Avenue, Des Moines, IA 50312. For questions, call (515) 271-1461. Make sure you are using the name Des Moines University. Hospitals may have difficulty locating our phone number if you are using a past name of the school.

I need a replacement diploma
You may order a replacement diploma by submitting a written request. There is a $50.00 charge, which may be paid by check or credit card. If paying by credit card, include both the credit card number and expiration date. All credit cards except Discover are accepted.
Include the following information in your request:
- Name, Program/Year
- Address
- Telephone Number
- Name to be printed on diploma
- Notarized signature
Your request may be faxed or mailed to our office. The fax number is 515-271-7060. The mailing address is Registrar's Office, Des Moines University, 3200 Grand Avenue, Des Moines, IA 50312.
Please allow 4 to 6 weeks for your diploma to arrive. If you have any questions, call us at 515-271-1460. Our office is open Monday through Friday from 8:00 a.m. to 4:30 p.m. central standard time. Voice mail is available if the office is closed.
